By implementing this Affiliate Agreement you are agreeing to the terms herein.

Kelly Brogan, MD will execute this agreement in good faith and without unnecessary modifications. Should it become necessary to alter this agreement in any way, we shall notify the Affiliate via email and specify these alterations. Affiliate may then agree to accept these changes or opt-out of Agreement. Opt-outs may be performed at any time by sending your request via email to [email protected].
